Fee for Service
We accept Medicaid, Medicare, and most Private Insurance plans. If you have insurance your insurance will be billed and you will be responsible for your co-pay.
Patients that do not have health insurance are eligible to receive services on a Sliding Scale. Patient must complete an application on site to determine discount. Discounts are offered depending on Household Income and Household Size.
If you are a new patient, and do not have health insurance, please bring the following documents with you at the time of your visit:
Proof of Income (ONE of the following):
W-2 Income Tax Withholding Form
Form 1040
2 Current Paystubs
Unemployment Insurance Book or Letter
Food Stamp Approval Letter
Social Security Benefits Letter
Veterans Benefits Letter
Letter from Employer on their letterhead stating annual income
If Self Employed, Tax Forms from current year and profit/loss statement |
Proof of Identification (ONE of the following):
Passport / Permanent Resident Card (I-551)
Driver’s License or State Issued ID
Birth Certificate
Military ID
Photo ID from Employer
Recent Utility Bill |
